Job summary

Enhabit Home Health & Hospice is seeking a licensed practical or vocational nurse to coordinate patient scheduling with field clinicians in Illinois. The role emphasizes communication with patients, physicians, and caregivers to ensure continuity of care and smooth scheduling workflows.

The ideal candidate possesses an LPN/LVN license, CPR certification, and at least one year of healthcare experience, with preference for Homecare Homebase (HCHB) experience and bilingual English/Spanish

Qualifications

• High School Diploma or equivalent.
• LPN/LVN license required.
• Valid clinical license per state requirements.
• At least one year of clinical healthcare experience.
• CPR certification required.
• Minimum one year of home health, hospice, or pediatric experience in last 24 months with staffing and scheduling knowledge.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including operation of a mobile device.
• Preferred Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish communication.
• In-office attendance required; remote work not allowed.
• Must be able to drive in all weather conditions.

Responsibilities

• Schedule patients to field clinicians.
• Coordinate care with patients, physicians, referral sources, caregivers and service providers.
• Maintain on-call notebook and hospitalization logs to enhance communication.

Responsibilities

Responsible for scheduling patients to field clinicians. Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care. Manage the on‑call notebook and hospitalization logs to enhance communication among stakeholders

Qualifications

• Required to have High School Diploma or equivalent.
• Required to be a licensed practical or vocational nurse.
• Required to have and maintain a valid clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
• Required to have minimum of one year of clinical experience in a healthcare setting.
• Required to have and maintain CPR certification, as required by position. or
• Required to have a minimum of one year of home health, hospice, or pediatric experience within the last 24 months with demonstrated experience and understanding of staffing and scheduling requirements related to home care services.
• Required to have and maintain a valid state driver license in the current state of residence
• Required to have and maintain automobile liability insurance, as required by law
• Required to have basic demonstrated technology skills, including operation of a mobile device.
• Preferred previous experience using Homecare Homebase (HCHB).
• Preferred bilingual (English/Spanish) with the ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
• Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
• Oral communication, written communication, fluency in English, active listening.
• Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
• Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
• Enhabit does not waive this essential function by permitting duties to be performed remotely on occasion to accommodate temporary personal needs, such as appointments, or to operate in crises beyond its control, such as during a natural disaster or pandemic
