Job summary

FeldCare Connects seeks a passionate Physical Therapist to provide high-quality home-based care to patients of all ages in Santa Barbara and surrounding areas. This Independent Contractor role lets you set your own schedule and select treatment locations.

You will conduct one-on-one sessions, document patient progress, and determine the plan of care. FeldCare Connects offers scheduling support, care coordination, and a streamlined Cliniconnects referral system to help manage referrals

Qualifications

• Current Physical Therapist license issued by the state Board of Physical Therapy.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone usage.

Responsibilities

• Deliver quality home healthcare to patients of all ages in Santa Barbara and surrounding areas.
• Observe, record, and report patient responses to treatment; adjust plan of care as needed.
• Perform one-on-one treatment sessions in patients' homes.
• Document patient progress efficiently via EMR during or after visits.
