Paradigm Home Health Companies in Texas seeks a dedicated Physical Therapist to implement care plans in collaboration with a physical therapist and care team. You will educate patients and families on home safety and medical equipment use, and you will communicate patient progress back to the team.

The role requires independence, motivation, and the ability to work without direct supervision while delivering high-quality home health care to patients in their homes across the service area.

Qualifications

• Associate degree in physical therapy required.
• Current BLS/CPR certification.
• Home health experience preferred.
• Valid Texas driver’s license and automobile insurance.
• Knowledge of WellSky EMR software is a plus.

Responsibilities
