Responsibilities

• Assess patients on admission and ongoing, covering physical, psychological, social, and age-related needs.
• Plan, provide and document patient and family education addressing barriers to learning.
• Evaluate and revise the care plan based on patient needs.
• Assign and delegate nursing care according to patient needs and staff capabilities.
• Collaborate with physicians and other disciplines on treatment.
• Participate in performance improvement activities and PI teams.
• Document care accurately per policies and procedures.
• Complete initial and annual competency checks as directed.
