Job summary

AbsoluteCare in Maryland seeks a Certified Registered Nurse Practitioner or Physician Assistant in Adult Medicine with acute care experience to provide on-site care for acute illnesses and exacerbations, and to help patients avoid unnecessary ER visits.

You will collaborate with the Integrated Care Team to coordinate care, educate patients on chronic disease management, and support transitions of care with timely documentation and follow-up.

Qualifications

• Licensed CRNP or PA in Adult Medicine with acute care experience.
• ACLS certification required or preferred.
• Strong problem-solving and time-management abilities with patient-centered approach.

Responsibilities

• Provide on-site clinical care for acute illnesses and exacerbations.
• Coordinate with Integrated Care Team to ensure comprehensive care.
• Educate patients on chronic disease self-management and medication adherence.
• Monitor high-risk patients and coordinate transitions of care to reduce readmissions.
