The University of Utah Health is seeking a Rural Trauma Care Coordinator to manage full life-cycle coordination of trauma care for rural patients. You will serve as a liaison between patients and the healthcare system, coordinating services across multiple teams and facilities to optimize outcomes.

Qualifications

• Two years of background experience in nursing, social work, or care coordination.
• Bachelor's degree in nursing preferred; related healthcare field also considered.
• Experience in care coordination, patient advocacy, case management, or healthcare services is highly valued.

Responsabilités

• Schedules patient appointments, tests and procedures and coordinates care across interdisciplinary teams.
• Responds to and directs patient communications via phone, email and other channels.
• Assesses escalated patient questions and coordinates care plans.
• Monitors outcome surveys to provide data on outcomes.
• Identifies patient needs and educates patients on the process for informed decisions.
