This position answersall patient questions and collaborates with members of aninterdisciplinary team to coordinate care and facilitate resourceand discharge planning to reach optimal outcomes for thepatient.

Bachelor's degree or equivalency (one year of education can besubstituted for two years of related work experience) in publichealth, health education, health promotion, management, behaviorscience, social science, social work or nursing with 2 yearsrelevant work experience or Medical Assistant withState of Utah, National Certification, or equivalency and 5 yearsof clinical experience.
Demonstrated ability to lead as well as function effectively as amember of a team and successfully promote change. Stronganalytical, organization and leadership skills. PC/Windows literatewith working knowledge of the following Microsoft applications:Outlook, Word and Excel. Demonstrated human relation and effectivecommunication skills.
This is a patient-sensitve position.
Working knowledge of quality improvement and project managementmethodologies, tools and techniques and principles of changemanagement. Demonstrated ability to effectively facilitate andmentor project teams preferably using QI methods (i.e., PDSA).Familiarity with electronic medical record (EMR)applications.
