Job summary

TN Voices is seeking a Family THRIVE Coach to provide peer-based coaching, care coordination, and resource navigation for rural families across the Rural THRIVE pathways. Travel within assigned rural areas is required, and services are delivered in community-based, in-person, telephone, and virtual settings.

Qualifications

• Must have a 4-year degree.
• Willingness to travel regularly within rural service areas.

Responsibilities

• Family Engagement and Peer Support using a family-driven approach.
• Engage children, adults, and families to build trust and reduce stigma.
• Establish supportive relationships while maintaining professional boundaries.
• Help families identify strengths, priorities, goals and barriers to health and stability.
• Promote family choice, self-advocacy, independence and resilience.
• Maintain regular contact with participants based on needs and program expectations.
• Complete Whole Family Assessments and collaboratively develop Health and Stability Plans.
• Regularly review progress and adjust goals and actions.
• Coordinate connections to primary and behavioral health services and other supports.
