Formación

• Bachelor's degree required.
• Provide peer-based coaching, care coordination, and resource navigation to families.

Responsabilidades

• Engage families using a strengths-based, family-driven approach.
• Develop individualized Health and Stability Plans with participants.
• Coordinate resources and facilitate warm handoffs and referrals.
• Travel throughout the rural service area to deliver in-person, telephone, and virtual services.
• Maintain supportive relationships while upholding professional boundaries.
