Job summary

The South Carolina Department of Health and Human Services is seeking a Rural Health Transformation Program Coordinator to manage day-to-day activities under the RHT program, including Connections to Care and other initiatives. This in-office role supports subrecipient oversight, reporting, and coordination with CMS requirements.

The role requires a Bachelor's in Public Health or related field, strong organization, communication, and travel statewide.

Qualifications

• Three (3) years of experience in program coordination, project management, or grants administration—preferably in health, human services, or rural program settings.
• Strong organizational and multitasking skills with attention to detail and deadlines.
• Excellent written and verbal communication skills.
• Proficiency in Microsoft Office Suite, SharePoint, and project management tools.
• Familiarity with state or federal health programs, Medicaid, or community-based healthcare delivery preferred.
• Ability to build and maintain collaborative working relationships with diverse partners.
• Ability to travel statewide.

Responsibilities

• Coordinates the planning, implementation, monitoring, and evaluation of RHT-funded initiatives and pilot projects; maintains timelines and reporting schedules.
• Serves as the primary point of contact for initiative stakeholders; facilitates meetings, webinars, and community sessions.
• Collects, reviews, and synthesizes data to support program performance measurement and reporting.
• Assists with budget monitoring, invoice tracking, and materials for leadership briefings and updates.
• Leads or supports other projects and participates in process improvements; occasional travel as required.
