CareSource is seeking a Rural Health Liaison to conduct outreach and serve as a resource for members, providers, and rural community organizations. The role leads education initiatives, supports policy development, and promotes health equity for rural populations.

Responsibilities include engaging stakeholders, maintaining CRM records, and coordinating events while collaborating with MCOs to address social determinants of health. Strong communication and cultural competency are required.

Qualifications

• Bachelor’s degree or equivalent years of relevant work experience is required
• Minimum of three years in health care, public health, or community-based organizations
• Familiarity with rural health communities and Medicaid is preferred

Responsabilités

• Engage with members, providers, and CBOs to share information and educate on health access
• Drive satisfaction and engagement of rural health partners
• Route complex questions to appropriate resources and ensure timely resolution (≤30 days)
• Maintain organized records and document interactions in CRM
• Support recruitment and contracting by building relationships with providers and hospitals
• Promote access to care by connecting rural members to services and resources
• Conduct outreach activities including presentations and educational materials
• Collect and analyze data on rural health needs to identify opportunities
• Contribute to policy development and implement innovative rural health solutions
• Foster cultural competency training for CareSource staff
• Provide resources to providers and CBOs to address gaps in care and health literacy
• Organize community town halls and advisory group meetings
