Arthur Center in Missouri is seeking a dedicated Community Health Navigator to support Rural Health Transformation across Audrain, Callaway, and Montgomery counties. You will screen residents, connect them with primary care, behavioral health, and social services, and guide them through complex care pathways.

This full-time role emphasizes strong community relations, data tracking in CIE/EHR, and ongoing follow-up to ensure services are completed and needs resolved. Travel locally is expected.

Qualifikationen

• Bachelor's degree in Social Work, Public Health, Human Services, Psychology required.
• Master's degree in Social Work or related field preferred.
• Experience in community health work, care coordination, patient navigation, or public health outreach.

Aufgaben

• Conduct screenings to identify clinical, behavioral, and social needs in rural residents.
• Manage referrals and warm handoffs across hospitals, FQHCs, behavioral health providers, and community partners.
• Provide care navigation and health education to support engagement and follow-through.
• Maintain structured follow-up and document all activities in digital systems.
