The CDC Foundation seeks a Rural Health Transformation Community Health Worker (CHW) Support and Partnership Coordinator to advance the Kentucky RHTP across public health and clinical settings. The role provides statewide TA, program design help, and workforce development to CHWs, supervisors, and pilot sites, promoting evidence-based practices and sustainable workflows.

Hybrid work is offered; this position aligns with KY Department for Public Health priorities and requires strong collaboration

Formación

• 5+ years of experience as a Community Health Worker, social worker, public health professional, or related field required (bachelor's preferred).
• Ability to communicate with diverse audiences, facilitate trainings, solve problems collaboratively and drive program implementation.
• Strong understanding of CHW certification and renewal requirements per Kentucky Office of Community Health Workers.
• Ability to facilitate productive conversations, navigate conflict and build consensus among diverse stakeholders.

Responsabilidades

• Provide technical assistance to organizations implementing RHTP CHW initiatives, including design, workflow and onboarding.
• Promote integration of CHWs into clinical/public health settings with evidence-based practices and sustainable workflows.
• Deliver ongoing coaching and implementation support to CHWs, supervisors and pilot site leadership.
• Conduct regular site visits to assess progress and provide hands-on support.
• Promote fidelity to Kentucky CHW scope of practice and related regulations.
