Job summary

Walsh Hospital District Healthcare Center in Colorado is seeking a Nurse Practitioner to deliver accessible primary and acute care across Walsh Medical Clinic, telehealth, and on-site at Walsh Healthcare Center and Maplewood Homes. You will provide preventive care, manage chronic diseases, and treat acute illnesses with an emphasis on patient-centered outcomes.

You will participate in on-call telehealth support, collaborate with interdisciplinary teams, and use EHR and telehealth tools to

Qualifications

Current, unrestricted NP license in Colorado.
DEA license with full prescriptive authority.
Eligibility to provide services through the District's clinics and telehealth.
BLS certification required.
ACLS, PALS, NRP recommended.

Responsibilities

Provide primary and acute care during in-person visits and telehealth.
Deliver preventive care, wellness visits, and chronic disease management.
Evaluate and treat acute illnesses and injuries; provide diagnostics and med management.
Support on-call residents via telephone consultation and telehealth.
Travel between Walsh Clinic, Walsh Healthcare Center, and Maplewood Homes as needed.
Document in EHR and collaborate with interdisciplinary teams.
