Job summary

Purchase District Health Department seeks a Community Health Worker for the Rural Community Hub in the Purchase Area. You will connect partner organizations, support recruitment, coordinate across sectors, and bring frontline insights to Hub planning and implementation.

You will build collaborations among healthcare, social services, education, housing, and other community systems to improve health outcomes and quality of life for Kentuckians.

Qualifications

• Bachelor's degree or equivalent lived/professional public health experience preferred.
• Experience serving rural communities preferred.
• Strong written and verbal communication skills preferred.
• 4+ years in community health work or related roles preferred.

Responsibilities

• Build relationships with partner organizations and raise Hub awareness.
• Support partner recruitment and community outreach at local events.
• Gather partner input and elevate perspectives to Hub leadership.
• Facilitate connections across healthcare, social services, and community resources.
