Job summary

CommWell Health in rural North Carolina seeks a licensed or license-eligible mental health professional to join a mission-driven care team. You will conduct assessments, develop collaborative treatment plans, and provide individual, group, and family therapies using evidence-based practices.

You will maintain thorough documentation, meet licensure requirements, and contribute to culturally sensitive programs while supporting patient self-management and care coordination with colleagues.

Qualifications

• Master’s degree in counseling, psychology, social work, or related human services field.
• Licensure or eligibility for relevant mental health licensure within 24 months of hire.
• CPR/AED/First Aid or BLS plus Basic First Aid certification from recognized providers.
• NCI+ Preventive Certification.

Responsibilities

• Conduct clinical assessments, diagnose within scope, and develop collaborative treatment plans.
• Provide individual, group, and family interventions using evidence-based practices and ethical guidelines.
• Maintain timely, compliant documentation and meet supervision, training, and licensure requirements.
• Contribute to culturally sensitive therapeutic programs, participate in committees, and follow policies.
• Engage as an active team member, supporting patient self-management goals and case reviews.

Education

Master’s degree in counseling/psychology/social work or related field

Licensed or eligible for LPA/LMFT/LCSW/LCAS/LPC/LCMHC within 24 months

CPR/AED/First Aid certification or BLS + Basic First Aid

NCI+ Preventive Certification

Experience with underserved or rural populations

Familiarity with team-based care or PCMH/PCHH models

One year of clinical experience in primary care/community health/outpatient
