Job summary

Pleasant Acres Nursing & Rehabilitation Center is seeking a dedicated RNAC/LPNAC to join our team. You will lead resident assessments (MDS) and help develop care plans that meet regulatory standards and patient needs.

Qualifications

• Current RN or LPN license is required.
• Experience in healthcare, long-term care or rehabilitation preferred.

Responsibilities

• Develops and coordinates resident MDS assessments per current rules.
• Maintains and updates policies for MDS and care plans.
• Develops and maintains QA program for assessments and care plans.
• Assists in completing discharge portions of the care plans.
• Participates in facility surveys conducted by government agencies.
• Develops preliminary and comprehensive nursing assessments.
• Audits documentation for standard of practice.
• Audits MDS and care plans for accuracy and completeness.
• Notifies DON and administrator of problems promptly.

About the Role

• Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines t


• Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.


• Develop, implement, and maintain an ongoing quality assurance program for resident assessment/care plan.


• Assist in completion of the discharge portion of the care plan.


• Participate in facility surveys (inspections) made by authorized government agencies


• Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules, regulations and facility policies.


• Audits documentation for standard of practice.


• Auditing the resident assessment (MDS) and care plan for completeness, accuracy, and comprehensiveness.


• Audits the CMI for accuracy.


• Notifies the DON and administrator of problems in a timely manner.

RNAC/LPNAC Requirements

• Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)


• Prior experience in a healthcare setting; experience in long-term care or rehabilitation preferred
