Pleasant Acres Nursing & Rehabilitation Center is seeking an RNAC/LPNAC to join our care team in Pennsylvania. You will coordinate resident assessments (MDS), help develop care plans, and participate in quality assurance activities to ensure high standards of resident care.
Applicants must hold current RN or LPN licensure and have healthcare experience, preferably in long-term care or rehabilitation. This role emphasizes regulatory compliance, teamwork, and compassionate resident-focused care.

As a RNAC/LPNAC , you will play a vital role in delivering high-quality care to our residents. Your responsibilities will include:
Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines t
Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
Develop, implement, and maintain an ongoing quality assurance program for resident assessment/care plan.
Assist in completion of the discharge portion of the care plan.
Participate in facility surveys (inspections) made by authorized government agencies
Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules, regulations and facility policies.
Audits documentation for standard of practice.
Auditing the resident assessment (MDS) and care plan for completeness, accuracy, and comprehensiveness.
Audits the CMI for accuracy.
Notifies the DON and administrator of problems in a timely manner.

Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
