Job summary

Pleasant Acres Nursing & Rehabilitation Center is seeking a compassionate LPN Per Diem to join our dedicated care team. You will provide direct patient care under the supervision of a registered nurse, administer medications, monitor vital signs, and help develop individualized care plans for residents.

Qualifications

• Current LPN license in Pennsylvania.
• Experience in long-term care or rehabilitation preferred.
• Ability to provide direct patient care under RN supervision.

Responsibilities

• Provide direct patient care, under the supervision of a registered nurse, to residents with a variety of needs and conditions.
• Administer medications and treatments as prescribed by the physician.
• Monitor and record vital signs, and report any changes to the registered nurse.
• Develop and implement individualized care plans to meet the unique needs of each resident.
• Collaborate with the interdisciplinary care team to ensure comprehensive care.
• Participate in quality improvement initiatives to ensure the highest level of care.

We are seeking a highly skilled and compassionate LPN Licensed Practical Nurse Per Diem to join our team!

LPN Licensed Practical Nurse Responsibilities:

• Provide direct patient care, under the supervision of a registered nurse, to residents with a variety of needs and conditions
