Job summary

The role supports clinical triage, schedules follow-up, and helps remove barriers to healing in collaboration with wound/ostomy and primary care teams.

The position requires a BSN or health-related degree, current RN licensure in Utah, and CWCN certification is preferred.

Qualifications

• Bachelor's degree in Health Care Administration, Health Science or related field, or equivalency.
• One year of related nursing experience.
• RN licensure in Utah or eligibility to obtain within required timeframe.

Responsibilities

• Coordinate patient care plans across care teams and disciplines.
• Assist with discharge planning and follow-up after hospitalization.
• Coordinate specialty referrals and appointments.
• Provide education and triage to ensure timely care.
