Job Summary and Qualifications

• The Wound and Ostomy Center nurse is a licensed professional responsible for caring for patients with complex wounds, skin issues, ostomies, fistulas, or percutaneous tubes.
• Advocate for patients to achieve wellness and independence.
• Consult with the healthcare team on wound and ostomy management.
• Follow established policies to assess, treat, document, and educate patients, ensuring informed consent and safety.
• Coordinate outpatient and inpatient services, perform diagnostic assessments like ankle-brachial index, and employ infection control techniques.
• Monitor lab values, assist with procedures, and facilitate referrals.
• Communicate effectively with patients and families about care plans.
• Participate in quality improvement, education, and safety protocols.
• Maintain equipment, documentation, and confidentiality standards.
• Contribute as a clinical resource and participate in hospital committees and projects.

Qualifications Needed:

• Two to four years of wound and ostomy experience.
• Current RN license in NH or a compact state.
• BLS certification.
• Wound and/or ostomy certification (WOCN, CWCN, CSW, OMS, or WCC).
