Job Details
  • Job Title: Clinical Resource Coordinator
  • Department: Wound Care
  • Location: Massena Hospital
  • Hours: 40 hours/week, 8 am - 4 pm
  • Sign-on Bonus: $10,000
Summary

The Clinical Resource Coordinator is a registered nurse responsible for managing and delivering care to patients and families in outpatient practice settings. Acts as a team lead and resource for clinical staff, assisting in planning, directing, and evaluating care programs.

Responsibilities
  1. Lead and coordinate outpatient practice programs and services.
  2. Develop and implement care plans in collaboration with providers.
  3. Prioritize patient scheduling to ensure timely care.
  4. Serve as a liaison among physicians, patients, and clinical teams.
  5. Support and delegate to clinical staff.
  6. Provide patient and family education and support.
  7. Coordinate care with community resources and ensure documentation.
  8. Promote preventative care and reduce hospital readmissions.
  9. Participate in team meetings and maintain professional development.
Care Delivery
  1. Maintain safe nursing care and triage patient calls.
  2. Assist with procedures, medication reconciliation, and patient care.
  3. Coordinate follow-up care and lead team discussions.
Quality & Regulatory
  1. Participate in quality improvement activities.
  2. Ensure compliance with standards and regulations.
  3. Promote safety and accreditation standards.
People
  1. Mentor staff and provide education.
  2. Maintain excellent communication with patients and team members.
Qualifications
  • Graduate of an accredited nursing program.
  • Licensed RN in New York State.
  • Current BLS certification; ACLS preferred.
  • 3-5 years clinical experience; wound care experience preferred.
Physical Requirements

Medium work; includes walking, standing, and lifting up to 50 pounds as needed.

Salary Range

$66,000 - $84,200 annually.
