General Summary

Under the direction of the Director of Nursing of the Transitional Rehab Unit, the registered nurse coordinates all aspects of care for patients, fostering a therapeutic environment that promotes optimism, encouragement, and praise for positive behavior and goal achievement.

Responsibilities

Coordinate patient care across the adult to geriatric age groups, applying principles of growth and development across the lifespan. Assess patient data, interpret information to identify individual requirements, and deliver appropriate care. Lead team members (LPN, CNA/NA, Unit Secretaries) and ensure compliance with Nursing Department policies and procedures.

Education

• Graduate of an accredited Nursing Program.

Experience

• Minimum two (2) years of satisfactory experience in a long‑term care setting (preferred).

Special Employment Requirements

• Current license as a Registered Nurse issued by the Pennsylvania State Board of Nurse Examiners.
• Valid CPR certification.
• Clearance by Pennsylvania State Police criminal history check.
• Demonstrated ability to interact and communicate effectively verbally and in writing.
• Knowledge and skills to provide individualized care suitable to age, need, and culture of patients.

Physical Demands

• Requires exerting 25 to 50 lbs of force occasionally (10–33% of shift), 10–25 lbs of force frequently (33–66%), and less than 10 lbs constantly for routine tasks.
• Bending, reaching, stooping, and lifting throughout shift.
• Ability to self‑transport throughout facility.

Health Hazards

• Occasional exposure to blood and bodily substances.
• Potential exposure to needle sticks.

Working Conditions

Works in well‑lit and ventilated areas including clinic, nurse stations, patient rooms, treatment areas, dining rooms, etc.
