Four Seasons in North Carolina is seeking a Weekend Support RN to provide nursing coverage for patients, families and community partners during Friday–Sunday shifts. The role includes admission support and close coordination with the Clinical Team Leader to ensure a seamless hospice experience.

The ideal candidate has two years of clinical experience, a valid NC RN license, and strong communication and organizational skills.

Position Summary

Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.

Essential Duties & Responsibilities

• Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
• Serve as first responder for symptom-based calls from current patients after normal office hours
• Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
• Participate in implementation and coordination of patient’s hospice care
• Assist with emergency admissions
• Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
• Record and report the results of the nursing assessment.
• Plan, initiate, deliver, and evaluate appropriate nursing acts.
• Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
• Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
• Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regime.
• Provide teaching and counseling about the patient's health.
• Report and record the plan for care, nursing care given, and the patient's response to that care.
• Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
• Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.

Minimum Qualifications

Experience

• Two years of clinical experience preferred

Licensures & Certifications: (if applicable)

• Valid NC RN license
• Valid Driver’s License and proof of automobile insurance
