Job summary

Four Seasons is seeking a Weekend Support RN to provide nursing coverage to patients, families and community partners by phone and in-person visits, including hospice admissions as needed.

The position is full-time (36 hours/week) and reports to the Clinical Team Leader. It offers competitive benefits, PTO, tuition reimbursement, and employer-sponsored insurance, with a weekend schedule Saturday through Monday in Western North Carolina.

Qualifications

  • Two years of clinical experience preferred.
  • Valid NC RN license.
  • Knowledge of HIPAA privacy rules and regulations.

Responsibilities

  • Provide nursing coverage to patients and families via phone and in-person visits, including hospice admissions as needed.
  • Serve as first responder for after-hours symptom-based calls.
  • Coordinate with case managers and other health providers to implement patient-centered plans.
  • Document nursing assessments, plans and patient responses.
  • Teach, supervise and evaluate staff performing nursing functions.

Position Summary

Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.

Essential Duties & Responsibilities
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
  • Serve as first responder for symptom-based calls from current patients after normal office hours
  • Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
  • Participate in implementation and coordination of patient’s hospice care
  • Assist with emergency admissions
  • Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
  • Record and report the results of the nursing assessment.
  • Plan, initiate, deliver, and evaluate appropriate nursing acts.
  • Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
  • Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
  • Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
  • Provide teaching and counseling about the patient's health.
  • Report and record the plan for care, nursing care given, and the patient's response to that care.
  • Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
  • Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.

  • Must be willing and able to care for patients with infectious disease processes unless approved accommodations
  • Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
  • Knowledge of HIPAA privacy rules and regulations

Minimum Qualifications
Experience:
  • Two years of clinical experience preferred
Licensures & Certifications:
  • Valid NC RN license
  • Valid Driver’s License and proof of automobile insurance
