Job summary

Qualifications

Responsibilities

• Assess patient physiologic health status and communicate care plan.
• Collaborate with physicians and health team to coordinate treatments.
• Administer medications and monitor patient responses; document care.
• Provide patient and family education and discharge planning.
• Identify and respond to changes in patient condition and safety risks.
• Participate in planning for discharge and referrals as needed.
