Job summary

Maxim Healthcare is seeking Registered Nurses (RN) in Morgan County to provide in-home care visits for home health patients. The role includes quick 35 minute to 1-hour visits at the patient’s home with med passes, insulin assistance, wound care, and more.

Qualifications

• Active OH RN license.
• Current Health Certificate as required by facility contract or state regulation.
• Current PPD or Chest X-Ray and BLS required.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient care in home health setting.
• Initiates plan of care and communicates changes to prescribing physician or supervisor.
• Delivers nursing actions including therapeutic, preventive, and rehabilitative care.
• Intervenes for changes in patient condition and documents outcomes for the care team.
• Administers medications and treatments as prescribed and tracks effectiveness.
• Reviews plan of care and coordinates education, care coordination, and discharge planning.

Responsibilities:

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required


• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change


• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals


• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor


• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention


• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements:

• Current Registered Nurse (RN) for the state in which the nurse practices.


• Current Health Certificate (per facility RN contract or state regulation).


• Current PPD or Chest X-Ray.


• Current BLS card.
