Position Overview

Horizon Home Care and Hospice is seeking experienced nurses to join our top‑notch Home Care Team as PM Visit Nurses. The role involves ensuring delivery of quality home‑care services and coordinating care in line with our philosophy and standards.

Responsibilities

• Assess patient conditions and family or caregiver's conditions accurately and completely during each visit (physiological, psychosocial, and/or spiritual).
• Identify and provide for patient and family's educational needs.
• Provide patient care per physician orders and agency policy.
• Complete all required patient and agency paperwork accurately, thoroughly, legibly, and timely.
• Transcribe, implement and evaluate new physician orders accurately.
• Ensure staff schedules, payroll, and daily logs are accurate, legible, and timely.
• Work cooperatively with patients, caregivers, family members and agency staff.
• Communicate patient’s issues and concerns to the case manager, appropriate health‑care professional, and/or clinical manager promptly and accurately.

Work Schedule & Compensation

2nd shift, Monday‑Friday (2:30 pm – 11:00 pm). No weekends, but includes a holiday rotation. Per visit, exempt.
