Job summary

Horizon Home Care and Hospice is seeking a Weekend Visit Nurse (Baylor RN) to join our home care team in Wisconsin. You will assess patient conditions and coordinate services to reflect our nursing standards, ensuring quality care for assigned patients.

The role requires an active Wisconsin RN license, 1–2 years of Med/Surg or Home Care experience, and a reliable vehicle with a valid Wisconsin driver’s license. Weekend shifts from 8:00am to 8:00pm, with holiday rotation, are included.

Qualifications

• BSN preferred and licensed RN in Wisconsin.
• 1–2 years of Med/Surg or Home Care experience.
• Valid Wisconsin driver's license with reliable vehicle.

Responsibilities

• Assess patient conditions and family needs during each visit.
• Provide patient care per physician orders and agency policy.
• Transcribe, implement and evaluate new physician orders accurately.
• Complete all required patient and agency paperwork accurately and timely.
• Admit patients in a timely, thorough and accurate fashion.

As a Weekend Visit Nurse (Baylor RN) you willA ensure the delivery of quality home care and services as well as the coordination of services that reflect the philosophy and standards of home health and nursing care of assigned patients.A Provides services at quality levels consistent with professional standards of practice.

• Assess patient conditions and family / caregiver's conditions accurately and completely during each visit (conditions may include physiological, psychosocial and / or spiritual).
• Identify and provide for patient and family's educational needs.
• Provide patient care per physician orders and Agency policy.
• Complete all required patient and Agency paperwork in an accurate, thorough, legible and timely fashion to reflect a clear picture of care provided.
• Transcribe, implement and evaluate new physician orders accurately.
• Perform and document admissions in a timely, thorough and accurate fashion.

• Degree from an Accredited School of Nursing (BSN preferred)
• Current WI RN license

Every Saturday and Sunday, 8:00am - 8:00pm. Includes a holiday rotation.
