Valor Healthcare is seeking a passionate Registered Nurse to join our SE San Antonio Community Based Outpatient Clinic. The role involves assisting practitioners, conducting exams, and performing clerical duties with a focus on patient care and veteran service. A weekday schedule and competitive benefits are offered.

Requirements include BSN (or ADN per VA contract), 2–3 years in primary or ambulatory care, VA credentialing, active U.S. license, BLS and ACLS, and strong EMR skills.

Qualifications

• BSN required; VA contract may allow ADN.
• Minimum 2–3 years RN experience in primary/ambulatory care; government setting preferred.
• Must be credentialed and in good standing with VA.
• Must hold an unrestricted RN license in the employment state.
• Current BLS and ACLS certifications.
• Proficient in English; strong computer/EMR skills.

Responsibilities

• Assist practitioners with patient care, physical exams and tests.
• Triages unscheduled patients and assists in care planning.
• Act as point person within the PACT Team and co-managed care with VA/non-VA providers.
• Provide patient and family education on self-management, prevention and wellness.
• Monitor discharge lists and follow up with patients.
• Maintain confidentiality of veterans’ data and information.
• Comply with VA and company training requirements.
• Participate in outreach events and support clinic enrollment initiatives.
• Support patient enrollment and retention in appointments and nurse visits.
• Embrace and implement new clinical or operational initiatives.
