Job summary

Valor Healthcare, Inc. seeks a dedicated Registered Nurse to join the SE San Antonio Community Based Outpatient Clinic (CBOC). You will assist practitioners with patient care, exams, and tests within the PACT Team, utilizing VA guidelines to deliver high-quality care.

Qualifications

• BSN required; VA contract may allow ASN.
• 2-3 years RN experience in primary/ambulatory care; government healthcare environment preferred.
• Must be credentialed and remain in good standing with the VA.
• Unrestricted RN license in the state of employment or as required by VA contract.
• Current BLS and ACLS certification.
• Proficiency in written and spoken English.
• Strong computer skills, including EMR experience.
• Safety-sensitive position.

Responsibilities

• Assist practitioners with patient care, physical examinations, and ancillary tests.
• Triages unscheduled patients requesting care.
• Acts as point person within PACT Team.
• Assist in co-managed care with VA and non-VA providers and facilities.
• Provide patient and family health education focusing on self-management, prevention, and wellness.
• Monitor VA discharge list and follow up with patients.
• Maintain confidentiality of veterans' data and information.
• Comply with VA and company training requirements.
• Fulfill compliance requirements of OIG, JC, EOC, and lab oversight.
• Support outreach events and enrollment initiatives.
• Support patient enrollment and retention in appointments, follow-up visits, or nurse visits.
• Embrace and support new clinical or operational initiatives.
