Qualifications

• Minimum 3 years of clinical practice experience required.
• At least 1 year in case management / care coordination preferred.
• Knowledge of case management processes and utilization review criteria.

Responsibilities

• Coordinate and lead the Centralized UM team across patient care continuum.
• Collaborate with discharge planning and care managers to optimize transitions.
• Ensure compliance with Medicare/Medicaid and utilization management standards.
• Mentor new staff and lead QA and performance improvement activities.
• Train staff on Epic UR screens and related software.
