Job summary

UI Health is seeking a Healthcare Utilization Review Coordinator II to coordinate patient services across care levels, coordinating with the care team to ensure safe, cost-effective, timely delivery of services. The role emphasizes relationship-based care and evidence-based practice within an interdisciplinary framework.

The position requires a BSN or higher, Illinois RN licensure, and 5 years of medical-surgical experience, with EPIC experience preferred.

Qualifications

• Graduation from an accredited professional nursing program.
• Current RN licensure in Illinois.
• Five years of Medical-Surgical experience.
• BSN (or higher) required.
• Current BLS/CPR and/or other unit-specific certifications.

Responsibilities

• Advocates for patients, families, and significant others and the community.
• Coordinates patient services with physicians, RNs, and the multidisciplinary team.
• Manages discharge planning and appropriate level-of-care decisions.
• Documents risk assessment and discharge info in the medical record.
• Collaborates to resolve delays in consults, treatments, or discharge planning.
