Job summary

NurseRemotely is seeking a registered nurse to provide age-appropriate care across a designated patient group using the nursing process. You will collaborate with physicians and the health team to coordinate procedures and treatments and to direct patient care on the unit.

This role requires thorough patient assessment—physiologic and psychosocial—development of care plans, implementation of treatments, and ongoing documentation.

Qualifications

• Graduate from an accredited School of Nursing; Associate’s Degree in Nursing required.
• Current license to practice as a Registered Professional Nurse in NY State and/or Connecticut.
• BLS required; specialized certifications as needed.
• Bachelor’s Degree in Nursing preferred.

Responsibilities

• Assesses patient’s physiologic health status including interviews, chart review, and questions.
• Assesses psychosocial health status and coping needs.
• Identifies patient outcomes and develops individualized goals.
• Formulates and prioritizes the plan of care; coordinates supplies, equipment, and medications.
• Implements the plan of care with safety, accuracy, and timeliness; documents actions.
• Participates in discharge planning and coordinate referrals as needed.
• Evaluates care effectiveness and documents outcomes.
• Collaborates with physicians and health team members in treatment planning and execution.
• Performs related duties as required.
