Responsibilities

• Assess patient needs across physical, psychosocial, cultural and spiritual domains via observation and records review.
• Develop and implement patient care plans and monitor outcomes with the care team.
• Demonstrate competency in IV therapy, wound care, medications, catheterization and specimen collection.
• Maintain patient charts accurately and use IT tools to communicate and support decision making.
• Advocate for patient safety, health, and rights through collaborative care.
• Educate patients and families, assessing learning needs and delivering effective instruction.
