Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking an experienced RN Unit Manager to oversee clinical operations on a nursing unit. You will lead and mentor RNs, LPNs, and CNAs to ensure the highest quality resident care in a fast-paced skilled nursing environment.

The ideal candidate will hold an active New York State RN license, have at least two years of long-term care experience, and demonstrate strong leadership, communication, and knowledge of NYSDOH and CMS

Qualifications

• Active NY RN license required.
• Minimum 2 years experience in a Skilled Nursing Facility or long-term care.
• Previous leadership or charge nurse experience preferred.
• Strong knowledge of NYSDOH and CMS regulations.
• Excellent communication, organizational, and leadership skills.
• EMR experience with PointClickCare preferred.

Responsibilities

• Oversee daily clinical operations of assigned nursing unit.
• Supervise, mentor, and evaluate nursing staff (RNs, LPNs, CNAs).
• Ensure compliance with NYSDOH, CMS, and facility policies.
• Participate in survey preparation, response, and follow-up.
• Conduct clinical audits and ensure accurate documentation.
• Coordinate care plans with interdisciplinary team.
• Monitor quality indicators, infection control, and resident outcomes.
• Address resident and family concerns promptly and professionally.
• Assist with admissions, discharges, and transfers as needed.
