Job summary

Graduate Post Acute is seeking an RN Unit Manager to lead nursing operations on a designated unit, ensuring safe, resident-centered care within a 150-bed Sub-Acute and Long-Term Care facility.

The role requires PA RN licensure, BLS/CPR certification, at least five years of nursing experience, and a minimum of two years in supervision. Strong clinical leadership and collaboration with interdisciplinary teams are essential.

Qualifications

  • Must possess a current, unencumbered active RN license in Pennsylvania.
  • Valid BLS Certification (CPR) required.
  • A nursing degree from an accredited college or university is preferred.
  • Minimum of five (5) years of nursing experience.
  • Minimum of two (2) years of supervisory experience in a nursing care center, hospital, or other healthcare setting.

  • Assume full responsibility and accountability for resident care on the assigned nursing unit.
  • Oversee and coordinate resident care to promote high physical, mental, and psychosocial functioning.
  • Ensure timely incident reporting with follow-up and communication to leadership.
  • Participate in facility committees and programs as directed by the Director of Nursing.
  • Assist in developing, implementing, and evaluating policies and procedures.
  • Provide clinical leadership, guidance, and support to nursing staff on the unit.
  • Monitor compliance with care plans, regulations, and quality standards to ensure resident outcomes.
  • Collaborate with interdisciplinary teams to support resident-centered care and continuous improvement.
