Responsibilities

• Develop, implement and update individual care plans.
• Complete admission and discharge assessments as needed.
• Assist with acute health changes and coordinate with physicians and families.
• Ensure compliance with federal, state, and company health/safety regulations.
• Audit documentation and care for quality control/compliance.
• Communicate resident condition changes to families and interdisciplinary team.
• Provide ongoing training, orientation and evaluations for staff; manage staffing schedules.

Heres What Youll Do for Our Residents & Facility:

Care Needs & Planning: Assist to develop, implement and continually update individual care plans, supplement hands on care needs

Assessments: Complete admission, discharge and other assessments as needed

Triage & Interventions: Assist to handle acute health changes, physicians' orders and collaboration with care providers & families

Regulatory Standards: Ensure compliance with federal, state, county & company health/safety regulations

Quality Assurance: Audit documentation & care for quality control/compliance

Family & Team Liaison: Communicates changes in resident conditions to families/providers, works collaboratively with the interdisciplinary team

Team Oversight & Mentorship: Provide ongoing training, orientation & performance evaluations for staff. Manage staffing schedules and assignments
