Job summary

Prevea Health is seeking a Registered Nurse (RN) Triage for a full-time role in Wisconsin. The RN will provide comprehensive nursing care in a multi-specialty clinic, perform assessments, coordinate care, and support patients across the clinic experience while upholding safety and high-quality care.

Collaborating with physicians and team members, the RN will manage patient education, follow-up, and documentation in the EMR, ensuring efficient workflows and an excellent patient experience.

Qualifications

• Associate's Degree from an accredited Nursing program Required
• Bachelor's Degree (BSN) from an accredited Nursing program Preferred

Responsibilities

• Performs patient assessments across clinical situations, escalating urgent concerns with providers.
• Delivers full range of nursing care including rooming patients, obtaining vitals, administering medications and vaccines, assisting with procedures, and supporting provider visits.
• Responds to patient questions, concerns, and symptom changes via phone, messages, and direct interactions, and coordinates care plans and referrals.
• Maintains accurate clinical documentation in the EMR, adheres to regulatory standards, infection control, privacy requirements, and participates in quality improvement.
• Collaborates with physicians, advanced practice providers, medical assistants, and staff to support coordinated workflows and clinic operations.
• Adapts to fluctuating patient volumes and changing clinic needs, maintaining supplies and a safe environment.
