Job summary

Life Care Center of Elyria seeks a Treatment Coordinator (RN) to provide primary skin care to patients under physician supervision in a post-acute setting. This full-time role requires an active RN license, wound care experience, and CPR certification.

The position entails collaboration with the nursing team, educating CNAs on skin care, and coordinating with physicians to adapt treatment plans, while maintaining patient confidentiality and adherence to Life Care standards.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college or university.
• Active RN license in good standing in the applicable state.
• Prior wound care experience preferred.
• One year geriatric nursing experience preferred.
• CPR certification required upon hire or during orientation.

Responsibilities

• Provide primary skin care to patients under the medical direction and supervision of physicians.
• Educate CNAs on proper positioning, training, and skin care.
• Evaluate patients’ needs and revise treatment plan with physician and interdisciplinary care team (IDT).
• Deliver quality nursing care and exhibit excellent customer service.
• Assist in the evacuation of patients.
• Communicate effectively on an interdisciplinary team.
• Maintain confidentiality and follow policies and compliance procedures.
