Responsibilities

• Coordinate pre-transplant evaluations.
• Assess patient health status.
• Serve as a communication link between physicians, patients, families and multidisciplinary team members.
• Maintain compliance with UNOS, OPTN, CMS, and organizational requirements.
• Complete transplant-related documentation, including patient listings and status changes.
• Collaborate with case management, financial coordinators, social workers to support patient needs.
• Maintain accurate and current patient records and transplant databases.
