The Transplant Coordinator is responsible for coordinating and managing the care of transplanted patients. This role provides clinical expertise, patient education, care coordination, regulatory compliance, and interdisciplinary collaboration to support kidney, kidney-pancreas, and pancreas transplant programs.

• Review laboratory results and communicate abnormalities to providers.
• Manage immunosuppressive medication adjustments per protocol.
• Triage patient calls and symptoms.
• Monitor adherence to follow-up schedules and medications.
• Maintain OPTN/UNOS-required documentation and reporting.

Qualifications

Required

• Current Registered Nurse (RN) license in the State of Louisiana.
• Bachelor of Science in Nursing (BSN) or higher.
• Minimum of three years of clinical nursing experience.
• Strong assessment, communication, organizational, and critical thinking skills.
• Ability to work collaboratively in a multidisciplinary healthcare environment.

Preferred

• Certification in transplant nursing (CCTC, CPTC, or equivalent).
• Experience with UNOS/OPTN regulations and reporting requirements.

Knowledge, Skills, and Abilities

• Knowledge of transplant patient management and regulatory requirements.
• Ability to coordinate complex care across multiple disciplines.
• Strong patient education and counseling skills.
• Excellent written and verbal communication abilities.
