Responsibilities include directing TCM staff, delegating within scope, serving as the clinical resource and escalation point, and ensuring high-quality discharge planning and care

Qualifications

• RN licensure in Kentucky required.
• Associate degree in nursing required; BSN preferred.
• Two years of clinical experience preferred, with emphasis in primary care, discharge planning, or hospital-based nursing.
• Leadership, preceptor, or supervisory experience preferred.

Responsabilités

• Lead and implement the Transitional Care Management Program for patients transitioning to home care.
