Job summary

LiveWell Homecare Agency, LLC is seeking a Registered Nurse for the Transitional Care Unit in a Level I Trauma Center. You will perform comprehensive assessments, develop care plans, and coordinate with physicians, therapists, and case managers to ensure safe, evidence-based patient care.

This role emphasizes patient-centered care, teamwork, and ongoing professional development in emergency care. A Nursing Diploma/ADN with RN license is required; Bachelor's degree and 2 years' experience

Qualifications

• Nursing Diploma or Associate’s Degree and RN license.
• Bachelor’s Degree in Nursing and at least 2 years of experience preferred.
• Strong critical thinking, assessment, and clinical decision-making skills.
• Ability to work in a fast-paced environment while maintaining safety and quality.

Responsibilities

• Provide direct patient care, including assessment, diagnosis, planning, implementation, and evaluation according to nursing standards.
• Collaborate with multidisciplinary team members such as physicians, therapists, case managers, and pharmacists to ensure optimal patient outcomes.
• Educate patients and families on managing illnesses, post-treatment care, and medication administration.
• Develop, implement, and document individualized care plans, communicating effectively across care teams.
• Promote patient safety, quality, and regulatory compliance, participating in ongoing professional development.
• Assess physical, psychosocial, and emotional needs, considering patient values, cultural, spiritual, and demographic factors.
