University Hospitals in Beachwood, OH is seeking a PRN RN Transitional Care Coordinator at UH Ahuja Medical Center. Typical hours are 7am-3:30pm. The role focuses on coordinating post-acute care, discharge planning, and ensuring safe transitions to the next level of care.

The coordinator collaborates with the care team, family, and community resources to develop and modify post-acute plans, remove barriers to follow-up care, and coordinate hand-offs to home care, SNF, IRF or other settings.

Qualifications

• Graduate of an accredited school of nursing (Required)
• Bachelor's Degree (Preferred)
• RN license in Ohio or Compact (Required Upon Hire)
• Knowledge of post-acute care and care-transition processes (preferred)
• Proficiency with care coordination and discharge planning tools (required)

Responsibilities

• Assess patient needs including prior functioning and support resources.
• Collaborate with care team and family to optimize post-acute care plans.
• Coordinate discharge planning across home care, SNF, IRF, or other facilities.
• Provide updates to medical team regarding plans for stay, discharge or transition.
• Assist with staff recruitment and orientation as needed.
