Benchmark Human Services is seeking a full-time Registered Nurse, RN Trainer, to support adults in Intensive Treatment Service Homes. You will provide healthcare coordination, train Direct Support Professionals in medication administration, and ensure high-quality health services for individuals served.

The RN will oversee health-related records, liaise with interdisciplinary teams, and respond to medical needs while ensuring regulatory compliance.

Qualifications

• RN with current New Hampshire license.
• Experience in community/long term nursing is preferred.
• Experience working with individuals with intellectual, developmental disabilities and/or mental illness.
• Valid Driver’s License.

Responsibilities

• Provide healthcare coordination and direct interventions for adults in residential program.
• Train and supervise Direct Support Professionals in medication administration and health care needs.
• Coordinate health services in accordance with state and federal regulations.
• Monitor health status, document assessments, and respond to health events promptly.
• Attend medical appointments as required and participate in health-related meetings and policy updates.
• Respond to emergency medical situations in a timely manner.
• Oversee pharmacy and medication needs, including reviews and prior authorizations.
• Order medical supplies from formularies and maintain health records per policy.
• Assist with transitions of care to hospital, nursing facilities, or other agencies.
