Job summary

Community Health Center Of Cape Cod in Bourne, MA is seeking a Nursing Team Lead to supervise the clinic operations and coordinate the workflow of medical services for a primary care team, delivering high quality patient care.

Responsibilities include leading nurses and clinical staff, scheduling coverage, conducting performance evaluations, coordinating patient care, and engaging in continuous quality improvement and staff development.

• Conducting comprehensive patient assessments, managing superior plans of care for the patient/family/care team.
• Assisting team in provision and coordination of patient care, including care coordination and follow up, panel coordination and management, medication reconciliation, barrier assessments, phone triage and patient and family education.
• Participates in QI activities with team and QI committee to consistently improve outcomes and patient health.
• Promoting a continuous learning environment for new and existing staff.
