Job summary

Signature HealthCARE, Liberty Care and Rehabilitation Center in Liberty, KY, is seeking an experienced RN to join a team dedicated to resident-centered care and quality of life. You will provide direct nursing care, supervise CNAs, and collaborate with the interdisciplinary team.

Responsibilities include medication administration, care planning, and ensuring regulatory compliance. Familiarity with EHR and MS Office is a plus. Competitive pay and benefits are offered.

Qualifications

• Current, active RN license in Kentucky state.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Familiarity with EHR systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs—ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides on shift.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the care team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate admissions, transfers and discharges per procedures.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
