Job summary

Liberty Care and Rehabilitation Center in Liberty, KY, is seeking a registered nurse to provide direct patient care, administer medications, and collaborate with an interdisciplinary team to deliver high-quality, resident-centered care.

Qualifications

• Active RN license in the state of practice (KY).
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• CPR/BLS preferred (American Heart Association or Red Cross).
• Experience in supervisory/charge nurse roles is a plus.
• Familiarity with LTC regulations and strong documentation skills.

Responsibilities

• Provide direct nursing care, including meds and wound care.
• Assist residents with ADLs ensuring dignity and safety.
• Supervise nursing assistants and medication aides on shift.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
