Job summary

Rockford Center in Newark, DE is seeking a Registered Nurse (RN) in Behavioral Health for a PRN position. The RN provides direct nursing care to patients as prescribed by the physician and works under the direction of facility nursing leadership, delivering age-specific care across children, adolescents, adults and seniors.

A two-week orientation is required. Schedule typically Monday through Friday, 8:30 AM to 5:00 PM, with PRN coverage as needed.

Qualifications

• Associates or Bachelor's degree in Nursing with current DE license.
• Two years of supervisory experience in psychiatric patient care preferred.
• Current CPR certification required.

Responsibilities

• Provide direct nursing care per physician orders and facility policies.
• Administer PRN meds, document rationale and effectiveness.
• Report changes in patient condition to nurse manager each shift.
• Notify family and physician for ER transport or incidents.
• Collaborate on individualized care plan with the team.
• Document care, findings, and patient responses.
• Perform treatments and therapies as ordered by physician.
• Maintain HIPAA confidentiality and respectful care.

Position Summary

The Registered Nurse provides direct nursing care to patients as prescribed by the physician and is under the direction and supervision of the appropriate facility nursing leadership. The RN provides nursing care specific to the age population served. Nursing activities are limited to those in compliance with the State Nurse Practice Act and facility policies and procedures.

Responsibilites

• Conducts ongoing and individualized nursing care duties as assigned and as dictated by the patient’s condition in accordance with physician orders and facility protocols.
• When administering PRN medications, assess for confirmed med for PRN, documents need (rationale) and its effectiveness in the medical record.
• Reports change in patient physical, mental status or episodes of seclusion or restraint to nurse manager or supervisor during each shift.
• Ensures family and physician notification is completed and documented if a patient is transported to Emergency Room, Injured, Falls or is Placed in Seclusion or Restraints.
• Collaborates with other team members in the development, implementation, and evaluation of an individualized plan of care.
• Performs treatments and therapies as ordered by physician in an appropriate and timely fashion.
• Documents patient care, findings, and psychosocial responses to nursing interventions.
• Documents PRN medications given, reason and effect
• Initiates emergency measures according to age appropriate CPR and client resuscitation protocols.
• Participates in emergency crisis interventions according to age appropriate techniques.
• Maintains confidentiality related to patient, family, client facility and staff in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Provides patient care in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights, and dignity are preserved.

Requirements

Qualifications

• Associates or Bachelor's Degree with an emphasis in Nursing. Strong communication skills, excellent customer service and organization skills, highly effective listening skills, and strong clinical skills.
• Two (2) years of related Supervisory experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.
• Current license to practice nursing in DE (or eligible to receive or renew).
• Current CPR
